Please have your child’s medical records, including vaccination records, sent or brought to the office prior to scheduling their first appointment. If you require any forms to be completed, please bring them with you. Please call the office directly and we will be more than happy to address any questions or concerns.

We offer convenient online self-scheduling for annual well visits as well as same-day sick visits. If you do not see an appointment that works in our self-scheduling system, give us a call because we may have more options available. Online appointments can be scheduled HERE.

Please schedule these appointments as far in advance as possible since they require extra time. A 24-hour notice is required for any cancellations. Sick visits are scheduled separately from routine visits. For this reason, we cannot always accommodate your first choice. If your child is sick on the day of a well visit appointment, we may reschedule the physical. This is to ensure a thorough evaluation as well as time to address any preventive care needs. If multiple concerns are brought up during the well visit, it may be necessary to schedule a follow-up appointment to accommodate all concerns in a thorough manner.

The patient portal is a secure online website that gives patients and families convenient, 24-hour access to their personal health information from anywhere with an internet connection. You can view your child’s health information in the portal, including: 

• Recent doctor visits  
• Discharge summaries  
• Medications  
• Immunizations  
• Lab results

Missed Appointment Fee: $40.00 
Failure to pay a copay at the visit: $20.00 surcharge $20 surcharge per sick office visit not cancelled on time. 

We require 12 hours advanced notice for cancellations. A $30.00 fee will be applied for any missed appointments that are not cancelled 12 hours prior. On occasion we have office emergencies, which may cause a backup in the schedule. If this happens, you may speak to the front desk to re-schedule.

Please call our answering service at 845-424-4444. if it is after hours. 

If your child requires medical attention after hours, call the office at 845-424-4444, and someone will return your call within the half hour.  If we do not return your call within the half hour, call us again. Please call during regular business hours for appointments, prescription renewals, and insurance or billing questions. However, for urgent concerns, we are available 24 hours a day, 7 days a week. If you need to cancel an appointment or leave a message for one of our staff, you may also leave a message with our answering service.
