Insurance Information
Please have your updated insurance card with you at all times. Lab work done in our office may be limited by your specific insurance plan. Review your own plan to avoid unexpected charges.
Copayments are required at each office visit as part of your contractual obligation with your insurance company. There is a $15 fee for each unpaid copayment at the time of the visit. We will bill secondary insurance.
Phone Calls
Calls are returned during workday hours. Let the receptionist know if your call is urgent. Lengthy discussions are best done at scheduled appointments.
Referrals
Please allow us three days advance notice if you need a referral. Most referrals can be faxed, but some must be picked up. For referrals, please leave a message on our referral line, 845-947-1645, and include the name of the doctor, the telephone number, fax number, the doctor’s NPI number, the date of the appointment and your current insurance information.
You can find specialists from pediatric cardiology to pediatric neurology by searching the online directory of Boston Children’s Health Physicians or by calling the particular specialty’s department.
Antibiotics Policy
If you think your child needs antibiotics, he or she must be examined by the pediatrician. This ensures your child gets the safest, most appropriate treatment.
Forms
Please allow us two to three days to fill out non-urgent forms for school, camp, day care, and prescription refills for daily medications.
