* HPV is a 3 shot series given 0, 2 and 6 months. IF started before age 15, only 2 shots are needed. Started at age 9.
** Lipid panel if age 9 and again at age 18. Can be done at other ages based on family history
*** Meningococcal vaccine is recommended by Four Seasons Pediatrics at age 11 and at age 16. Required for school entry in New York for 7th and 12th grade
**** TdaP is recommended by Four Seasons Pediatrics at age 11 years. Required for school entry at age 11.
***** COVID vaccine is recommended starting at 6 months of age. We recommend this vaccine for those at higher risk of COVID including age 6 months to 2 years, those with high risk conditions or those who live or who spend significant amount of time with those with high risk, or those who desire protection. Some of these are considered off label use, a common and accepted medical practice.

Please consider the importance of scheduling visits for certain shots:
- 1 month check up should be timed to be 4 weeks from the Hepatitis B if given in hospital
- 6 month check up should be on or after 6 months of age if during a time when flu shots are given
- 12 month check up should be on or after your child’s birthday
- 18 month check up must be 6 months or more from the 12 month check up to get the Hepatitis A vaccine
- We give the second dose of Varicella at 3 years of age (instead of giving with 3 other vaccines at age 4).
- 4 Year check up must be on or after the birthday
