At your first visit, we’ll help you get familiar with our services and ask about your child’s individual needs. We know each patient is different, and we strive to provide each child with personal, individualized attention.
Newborn Visits: We have saved spots in the schedule for newborn appointments. Once you have given birth and are being discharged from the hospital please call the office. We will get you scheduled to see a pediatrician within 24-72 hours of discharge. Please remember to bring your discharge paperwork to your first appointment.
New Patient Sick Visits: We require your child's full immunization record before we can see them. This is not optional. If we do not have it in hand before the appointment, we will have to cancel and reschedule for a later date.
New Patient Well Visits: Along with the immunization record, we also need a copy of the last well visit note from your child's previous pediatrician. This lets us pick up right where your last doctor left off.
How to get us your records
If you already have a copy of the immunization record or well visit want to upload it to us directly, you can use the Submit a Form button at the bottom of the screen.
If you need to request records from your previous pediatrician, please click the medical release button below. Complete that form and submit it to your previous pediatricians office. Please be aware it can take up to 30 days for them to send us the records.
