New Patient

We’re so excited to welcome you and your child to Middletown Pediatrics!

To become a patient of our office, please call 845-281-7667 and we will help you get started and answer any initial questions you may have.

At your child’s first visit, we’ll take time to get to know your family, review your child’s health history, and explain our services. We know that every child is unique, and our goal is to provide gentle, personalized care in a warm and supportive environment.

To help make your visit as smooth as possible, please bring:

  • Your child’s health insurance card or insurance information
    • A list of all medications your child is currently taking
      • A list of any questions or concerns you’d like to discuss
      • Copies of medical records from previous doctors

We look forward to meeting your family and partnering with you in your child’s health and well-being!

Well Visit Schedule

Regular well visits help us cheer on your child’s growth, track development, answer your questions, and keep immunizations up to date. These checkups are a great way to partner with your pediatrician and make sure your child is thriving, from tiny newborn moments to confident teen years.

We recommend scheduling well visits 4–8 weeks in advance to secure your preferred appointment time. If you have any concerns, especially during the first few months of life, your pediatrician may suggest more frequent visits. We’re always here when you need us.
